Overheating under towing/high load
Problems with the unit are most often caused by overheating from towing, power upgrades, or inadequate cooling capacity.

Limp-home gear default
If the TCM loses certain key inputs it de-energizes all solenoids, causing the transmission to hydraulically default to 3rd gear when stationary or 4th gear when moving.

Output shaft speed sensor failure
Failure of the output shaft speed sensor, which replaces the hydraulic governor, forces the transmission into a default gear.
