Loss of lubrication oil to the planetary gear sets can lead to failure of the reverse drum, piston retainer and planet assembly.

Burnt Reverse band
A worn or broken reverse servo piston plug can allow the reverse band to burn, causing loss of reverse gear.

OD roller clutch failure
The overdrive one-way (roller) clutch is a documented failure point, often worsened when aftermarket bushings fit poorly in the output shaft housing.
