No reverse after overhaul
After an overhaul or valve body/clutch R&R, the transmission fails to engage Reverse from Neutral or Park because the turbine and output speed sensor wiring was swapped, causing the TCM to inhibit Reverse.

Torque converter damage (abnormal stall speed)
A stall speed test result outside specification, with normal fluid color/odor, points to internal torque converter damage requiring converter replacement.

Limp-home stuck in 4th gear
When the TCM detects certain electrical faults it enters limp-home mode, substituting fixed values so the transmission stays in 4th gear in Drive (3rd in Low) until manually shifted.
